Born Joined at the Pelvis, These Baby Sisters Spent 15 Months Together — Then Their Mother Finally Got to Hold Them Separately
When Sam Albalushi learned she was expecting twin girls, she imagined the joy of holding two newborn daughters in her arms.
But when Ally and El arrived prematurely in November 2023, doctors discovered that the babies were physically joined at the pelvis.
Their shared anatomy made everyday care complicated and raised difficult questions about their future.
For 15 months, the sisters lived connected to one another. Then an extraordinary team of medical professionals began an operation that would change their lives forever.
A Mother’s Shock After Her Twins Were Born
During her pregnancy, Sam noticed that her babies were not moving as expected. At 33 weeks, doctors recommended a cesarean delivery after their movements decreased significantly.
When the girls were born, medical staff quickly took them for specialized care.
Sam learned that Ally and El were conjoined at the pelvis, a particularly uncommon form of conjoined twinning.
The girls had complex connections involving their lower abdominal and pelvic structures, making separation an exceptionally challenging possibility.
As their mother adjusted to the unexpected diagnosis, she remained determined to find the medical support her daughters needed.
Months of Preparation for Two Separate Lives
In 2024, Sam brought Ally and El to Seattle Children’s Hospital for a surgical consultation.
There, she met Dr. Caitlin A. Smith, co-director of the hospital’s Reconstructive Pelvic Medicine program, and other specialists.
Detailed preparations began in October 2024.
Doctors used advanced imaging, including MRI, fluoroscopy and endoscopy, to understand the girls’ anatomy and identify their shared structures.
A team of more than 30 healthcare professionals worked together to plan the operation.
Surgeons, anesthesiologists, nurses and other specialists held regular meetings to discuss every stage of the procedure, including the equipment and backup resources they might need.
An 18-Hour Operation That Changed Everything
On February 28, 2025, when the twins were 15 months old, the medical team began the long-awaited separation surgery.
The procedure required careful separation of the girls’ connected gastrointestinal, gynecologic and urinary systems.
Surgeons also worked through their shared pelvic anatomy before reconstructing the necessary structures for each child.
Absorbable mesh and specialized tissue reconstruction techniques were used during the final stages.
After approximately 18 hours, Ally and El were successfully separated.
For the first time since birth, the little sisters were no longer physically connected.
Two Separate Cribs and a New Beginning
Following surgery, the twins recovered in intensive care before moving to a regular hospital ward.
They initially faced movement restrictions while their surgical wounds healed.
For Sam, seeing her daughters recovering in separate cribs was an emotional milestone.
The girls then began rehabilitation, receiving physical, occupational and speech therapy to help them develop strength, movement and communication skills.
By late May 2025, both sisters were strong enough to leave inpatient rehabilitation and continue recovering at home.
Learning to Walk and Discovering Their Own Personalities
As Ally and El continued growing, their family began experiencing moments that had once seemed far away.
The sisters learned new words, crawled more confidently and worked on strengthening their muscles as they prepared to walk.
In a January 2026 interview, Sam shared that her daughters, then two years old, were beginning to take their first steps.
Their recovery continued to involve therapy and medical follow-up, but their progress brought enormous happiness to the family.
After spending their earliest months physically joined, Ally and El were now discovering their own movements, abilities and individual personalities.
For their mother, one of the most meaningful changes was being able to embrace her daughters separately.
The girls would always share an extraordinary bond as sisters. But thanks to months of preparation and an 18-hour operation, they could now explore the world as two individuals.
